• US citizen
• At least 18 years of age
• Residents of Alabama, Arkansas, Georgia, Florida, Louisiana, Mississippi, North Carolina, South Carolina, Tennessee, Texas, or Virginia

• Date of birth
• Monthly income
• Employer information
• State or government issued-ID
• Social Security number
